Provide direct rehabilitative services to patients in their homes, focusing on evaluating and treating speech, language, and dysphagia disorders. Develop and implement individualized plans of care to improve and restore clients' quality of life.
Responsibilities
- Comply with ethical and professional standards of conduct as set forth by the American Speech Hearing and Language Association
- Practice and maintain patient safety and dignity by abiding by Federal, State, Local, and Optimal policy & procedures as pertains to patient protected health information and HIPAA
- Evaluate patient speech and language abilities, including defects and assets
- Plan and provide rehabilitative services for speech, language, and dysphagia disorders
- Participate in the development of the Physician’s Plan of Care
- Develop, document, implement, and re-evaluate patients' Plan of Care
- Record clinical/progress notes to the patient’s clinical record weekly, detailing treatment type and patient’s reaction, progress, and/or regression
- Maintain adequate records on all patients
- Utilize community resources and agency personnel by proper referral
- Select and administer diagnostic and therapeutic techniques and materials
- Instruct and counsel other health team personnel and family members in methods of assisting patient in improving, correcting, and accepting their disabilities
- Plan for patient’s discharge, prepare discharge summaries and instructions
- Attend agency service meetings and participate in staff development activities
Requirements
- Master’s or Doctoral Degree in Speech-language Pathology or Audiology
- Currently licensed as a speech-language pathologist in good standing in the state of Colorado
- Ability to communicate in the language of the client and document client related activity
- Adequate and effective written and verbal communication skills
- Adequate and effective interpersonal skills
Qualifications
- Master’s or Doctoral Degree in Speech-language Pathology or Audiology
- Preferably one (1) full year of supervised work experience, experience in Home Health as an SLP preferred.
