Implement treatment programs for pediatric patients with cognitive, speech, language, feeding/swallowing, and social/emotional disabilities. Provide high-quality care and support to patients and their families within a home and community setting.
Responsibilities
- Provide high quality care and meet the needs of the patient and family by performing evaluations and interpreting assessment results
- Develop goals appropriate to child; create and implement speech therapy treatment plans in conjunction with the physician
- Assist pediatric patients to develop or regain cognitive, speech, language, feeding/swallowing and/or social/emotional functioning
- Implement skilled interventions to maximize the potential of each individual child
- Participate in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine
- Coordinate with referral partners to provide services for children in accordance with the physician order
- Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs
- Document patient care services and care coordination in an intuitive electronic medical record system
- Maintain patient confidence by keeping information confidential
Qualifications
- A minimum of 1 year of experience preferred
