About This Role

This Speech Language Pathologist (SLP) role involves implementing treatment programs for pediatric patients with cognitive, speech, language, feeding/swallowing, and/or social/emotional disabilities. You will provide high-quality care, perform evaluations, develop treatment plans, and educate caregivers in a home health setting.

Responsibilities

  • Provide high quality care and meet the needs of the patient and family by performing evaluations and interpreting assessment results
  • Develop goals appropriate to child; creating and implementing speech therapy treatment plans in conjunction with the physician
  • Assist pediatric patients to develop or regain cognitive, speech, language, feeding/swallowing and/or social/emotional functioning
  • Participate in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities
  • Coordinate with referral partners to provide services for children in accordance with the physician order
  • Assure continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs
  • Document patient care services and care coordination in an intuitive electronic medical record system
  • Maintain patient confidence by keeping information confidential

Requirements

  • Feeding experience

Qualifications

  • A minimum of 1 year of experience preferred
