About This Role

Join an interdisciplinary rehabilitation flexible workforce team as a Speech Language Pathologist. You will be responsible for providing safe and effective delivery of patient care within your scope of practice, including examination, evaluation, diagnosis, planning, intervention, and establishing outcomes in collaboration with patients and families.

Responsibilities

  • Partner with patients and families to perform evaluation and re-evaluation
  • Gather pertinent data and perform examinations
  • Determine treatment diagnosis
  • Develop individualized plan of care collaboratively with patient and family
  • Analyze data to identify needs and potential risks
  • Determine prognosis for recovery and establish individualized goals
  • Individualize plan of care considering developmental level and cultural differences
  • Implement and modify plan of care based on reassessment and patient response
  • Coordinate care and communicate effectively with interdisciplinary team
  • Establish appropriate discharge plan
  • Implement plan of care and alter treatment to reflect change in patient status
  • Assess patient and caregiver learning needs and provide appropriate education
  • Complete documentation in a timely manner per department standards
  • Comply with department billing standards
  • Prioritize tasks and patient care for effective time management
  • Understand and follow professional licensure/certification requirements
  • Direct team members and/or supervise students as designated
  • Pursue personal and professional development to support clinical best practice
  • Meet or exceed departmental standards for productivity
  • Identify opportunities for process/quality improvement
  • Participate in and support entity/system initiatives

Requirements

  • Certification by American Speech-Language-Hearing Association (ASHA)
  • Speech Language Pathology license in the state of Minnesota
  • Basic Life Support (American Heart Association or Red Cross)

Nice to Have

  • 1 year experience with adults and pediatrics
  • Feeding services experience and trained
  • Completion of CFY
