Social Worker, Hospital Care Management

About This Role

This role involves providing comprehensive social work services within a hospital setting, focusing on patient and family psychosocial risk factors, discharge planning, and crisis management. The social worker will collaborate with interdisciplinary teams to coordinate patient care and ensure appropriate post-hospital services.

Responsibilities

• Provide screening, assessment, planning, problem resolution, resource management, counseling and crisis management related to health care and discharge planning needs
• Manage and collaborate in discharge planning for patients with simple and complex needs, coordinating with agencies providing post-hospital care
• Function as an interdisciplinary health care team member, collaborating with professionals to assess patient/family needs and develop comprehensive care plans
• Assess patients’ and family’s psychosocial risk factors, prior functioning levels, support systems, and coping abilities
• Ensure documentation in the medical record is legible and conforms to hospital department policies and procedures
• Promote the Hospital Case Management team concept with discharge planning activities and maintain close communication with post-acute care providers
• Screen and coordinate all SNF and Rehab facility referrals, providing consultation to team members regarding clinical needs
• Advocate for patient and family empowerment and independence to make autonomous health care decisions and access needed services
• Intervene with patients and families regarding emotional, social, and financial consequences of illness and/or disability, mobilizing family/community resources
• Provide intervention in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse and sexual assault
• Serve as a resource person and provide counseling and intervention related to treatment decisions and end-of-life issues
• Participate in the development and implementation of policies and procedures for the Case Management program
• Assess the patient's behavioral health needs in collaboration with physician, psychiatrists, and other mental health team members
• Provide counseling, crisis intervention, and psychosocial assessments to patients/families with appropriate medical staff supervision

Requirements

• Master's degree in social work
• Currently licensed as a Licensed Master Social Worker (LMSW) or Licensed Independent Social Worker (LISW) by the state Board of Social Work Examiners

Qualifications

• Master's degree in social work
• One (1) year Social Work Experience preferred. One (1) year experience in a healthcare setting preferred.
