Registered Nurse (RN) - Acute Care, Transplant and Intermediate Care

About This Role

This role provides clinical direction for patient care in an acute inpatient setting, focusing on transplant and intermediate care patients. It involves assessing, planning, implementing, and evaluating nursing care utilizing evidence-based practices.

Responsibilities

• Collect and complete nursing assessments using appropriate standards and tools, recognizing changes in patient status
• Develop individualized patient care plans in collaboration with patients, families, providers, and healthcare teams
• Update care plans based on patient condition changes and communicate updates to team members
• Identify realistic and measurable patient goals and outcomes, validating them with the patient and/or family
• Provide direct care to assigned patients consistent with the established nursing plan and RN scope of practice
• Delegate nursing tasks as appropriate and ensure completion of assigned tasks
• Monitor quality of care delivered, ensuring appropriate surveillance of assigned patients
• Assume responsibility for patient safety by maintaining a safe environment and adhering to care standards
• Evaluate plan of care for effectiveness based on patient response, document progress, and revise as needed
• Identify and assess patient/family learning needs and provide appropriate education

Requirements

• Licensed to practice as a Registered Nurse (RN) in South Carolina
• ACLS certification
• BLS certification
• 1 year of Registered Nurse experience OR completion of a Nurse Residency Program OR 1 year of LPN experience for internal candidates

Qualifications

• Associate degree in Nursing. Bachelor's degree in Nursing preferred.
• One (1) year experience as a registered nurse, or completion of a Nurse Residency Program, or 1 year LPN experience for internal candidates.
