About This Role

Utilize counseling skills to coordinate the transition of court-adjudicated youth back into their communities by leading discharge planning, establishing aftercare networks, and conducting follow-up, ultimately creating positive changes in their lives.

Responsibilities

  • Coordinate the transition of youth at the facility back into their communities
  • Lead an active process of discharge planning
  • Establish an aftercare network of community-based resources
  • Conduct follow-up after youth leave the facility
  • Participate in the multidisciplinary process and assume responsibility for aftercare and discharge portions
  • Plan transitions with residents and their families
  • Identify the needs of the youth, their family, and social support system
  • Administer exit interviews
  • Attend court hearings when necessary
  • Coordinate with outside agencies
  • Participate in meetings, conferences, and training to develop and maintain position skills

Requirements

  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) or an equivalent Federal Government job title (if employed in a commonwealth social worker job title on September 8, 2008, with continuous commonwealth employment)
  • OR Two years of experience as a Social Worker 1 or a Forensic Social Worker 1 and a bachelor’s degree in social work or social welfare
  • OR Two years of professional experience providing direct public or private social work services, and a bachelor’s degree in social work or social welfare
  • OR A master's degree in social work or social welfare
  • Must meet PA residency requirement
  • Ability to perform essential job functions
  • Submission and approval of satisfactory criminal history reports including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance

Qualifications

  • Bachelor's degree in social work or social welfare or a Master's degree in social work or social welfare
  • Two years of experience as a Social Worker 1 or Forensic Social Worker 1 (commonwealth titles) or two years of professional experience providing direct public or private social work services.
