RN Discharge Lounge

About This Role

This role is for a Registered Nurse in the Admission and Discharge Lounge, focusing on providing exceptional patient-centered care and ensuring a seamless transition for patients entering and leaving acute care hospital settings, while advocating for patients, families, and caregivers.

Responsibilities

• Obtain a nursing assessment history and ensure proper inpatient care unit admission, taking appropriate action when needed.
• Complete and facilitate required documentation such as admission assessments, risk assessments, medication reconciliation, discharge paperwork, and post-hospital documentation.
• Teach patients, families, and caregivers about a patient’s medical condition, treatment plan, medications, and follow-up measures.
• Complete extra discharge documentation, education, and care for special patient populations to ensure compliance with core measures.
• Ensure patients, families, and caregivers receive copies of discharge paperwork and other necessary documentation like referrals and educational materials.
• Coordinate with House Supervisors/Nursing Administration to prioritize workload and identify greatest need areas for smooth patient transitions.
• Demonstrate time-conscious awareness and ability to prioritize, distinguishing between low and high-priority activities.

Requirements

• Registered Nurse license
• Associate Degree, Registered Nurse Diploma, or Bachelor's Degree
