About This Role

Coordinate and deliver high-quality, patient-centered care for rehabilitation patients with various diagnoses, utilizing established nursing models and collaborating with the healthcare team.

Responsibilities

  • Assess, plan and evaluate patient care needs
  • Administer prescribed medications, change dressings, clean wounds, and monitor vital signs
  • Monitor, record and communicate patient condition using computerized documentation systems
  • Assess and coordinate patient's discharge planning needs with members of the healthcare team
  • Provide age and culturally appropriate care

Requirements

  • Registered Nurse license
  • Associate Degree
