Provide holistic, quality, and cost-effective nursing care for patients in the pre-hospitalization population at Hartford Hospital. This role involves pre-screening calls, chart reviews, risk identification, and collaborating with providers for testing.
Responsibilities
- Provide holistic, quality and cost-effective nursing care for patients/families and/or others in the pre-hospitalization population
- Focus on discharge planning including identification of risk factors and education
- Apply the nursing process within the context of the organization's Framework for Professional Practice
- Function as clinically competent/expert
- Delegate and supervise work appropriately
- Complete Pre-Screening Calls
- Complete preoperative chart reviews in Epic
- Accurately obtain, assess, and identify potential risks and barriers to expected outcomes
- Provide patient care in partnership with a provider; including phlebotomy, EKG, and other preoperative testing as ordered
Requirements
- CT RN License
- BSN Required
- 1 year RN experience
