Registered Nurse

About This Role

Phoenix Home Care and Hospice is seeking a Registered Nurse for private duty care and VA departments, focusing on in-home visits and Healthy Children & Youth program assessments. This role involves providing high-quality care to clients in their homes, including aide oversight and routine nursing tasks.

Responsibilities

• Provide basic patient care including taking vitals, weekly medication management per physician orders, monthly diabetic nail care, monthly skin monitoring, and bi-yearly general health assessments.
• Open new cases and obtain all related consents, documentation, and deposits in accordance with company policy.
• Observe, record, and report reactions to treatment and any changes in client's condition to appropriate personnel and/or physician.
• Supervise In Home Aides for Advance Personal Care (APC) Services.
• Provide instruction and education on medication when needed.
• Follow state-approved care plans.
• Attend quarterly Nurse Meetings.

Requirements

• Active Registered Nurse license in the state of MO
• Valid driver's license
• Strong organizational and self-management skills
• Strong and compassionate customer service skills

Qualifications

• In-Home experience preferred.
