Provide direct nursing care to patients by implementing and monitoring care plans, coordinating with healthcare teams, and ensuring individualized care in accordance with established policies and protocols.
Responsibilities
- Implement and monitor patient care plans
- Monitor, record, and communicate patient condition as appropriate
- Serve as a primary coordinator of all disciplines for well-coordinated patient care
- Note and carry out physician and nursing orders
- Assess and coordinate patient's discharge planning needs with members of the healthcare team
- Demonstrate the knowledge and skills necessary to assess patients and to identify physical, psychosocial, cultural, spiritual and age-related needs in order to provide individualized care to the patient
- Delegate aspects of care to other nursing personnel consistent with their level of education and expertise
- Participate in daily interdisciplinary patient rounds
- Develop, implement and revise individualized plans of care
Requirements
- Graduate of an accredited nursing school
- Associate degree
- Current Connecticut RN license
- BLS certification by the American Heart Association
- Excellent assessment and critical thinking skills
Qualifications
- Associate degree required; BSN preferred
