About This Role

Manage Home Hemodialysis (HHD) patients for DaVita Kidney Care. This role involves educating patients and caregivers to perform dialysis independently, blending clinical care, patient education, and case management.

Responsibilities

• Train patients and caregivers on HHD procedures and self-care
• Manage a caseload of HHD patients, including assessments and care planning
• Monitor treatment progress, conduct home visits, and provide on-call support
• Collaqborate with interdisciplinary teams to ensure safe, high-quality outcomes

Requirements

• RN license in the state of practice
• Valid driver’s license, insurance, and reliable transportation
• ADN
• Basic computer skills (MS Word, Outlook)

Qualifications

• ADN required; BSN preferred

Nice to Have

• Dialysis (HHD, PD, chronic, or acute) experience
• Patient education or teaching background
• Case management or telehealth experience
• CNN or CDN certification
• BSN
