About This Role

Manage and educate Home Hemodialysis (HHD) patients and their caregivers to empower them to perform dialysis independently at home. This role involves clinical care, patient education, and case management, ensuring safe and high-quality outcomes.

Responsibilities

• Train patients and caregivers on HHD procedures and self-care
• Manage a caseload of HHD patients, including assessments and care planning
• Monitor treatment progress, conduct home visits, and provide on-call support
• Collaborate with interdisciplinary teams to ensure safe, high-quality outcomes

Requirements

• RN license in the state of practice
• Valid driver’s license, insurance, and reliable transportation
• ADN required
• Basic computer skills (MS Word, Outlook)

Qualifications

• ADN required; BSN preferred

Nice to Have

• Dialysis (HHD, PD, chronic, or acute) experience
• Patient education or teaching background
• Case management or telehealth experience
• CNN or CDN certification
