About This Role

Provide exceptional home health care to patients in their homes, conducting assessments, developing treatment plans, and delivering hands-on nursing care. Specialize in acute and chronic disease management, medication management, and post-surgical rehabilitation support for patients.

Responsibilities

• Conduct in-home OASIS assessments, evaluations, and develop personalized, goal-driven treatment plans
• Provide clinical oversight and guidance to Licensed Practical Nurses (LPNs)
• Collaborate with patients, caregivers, and a supportive clinical team to deliver exceptional care
• Utilize dictation services to streamline documentation and maximize time for patient care
• Deliver hands-on nursing care, including acute & chronic disease management
• Manage patient medication
• Provide post-surgical rehabilitation support
• Administer pain management
• Perform wound care

Requirements

• Unencumbered Florida Registered Nurse license
• Valid driver’s license
• Auto insurance
• Reliable transportation
