Provide exceptional home health care to patients in their homes, conducting assessments, developing treatment plans, and delivering hands-on nursing care. Specialize in acute and chronic disease management, medication management, and post-surgical rehabilitation support for patients.
Responsibilities
- Conduct in-home OASIS assessments, evaluations, and develop personalized, goal-driven treatment plans
- Provide clinical oversight and guidance to Licensed Practical Nurses (LPNs)
- Collaborate with patients, caregivers, and a supportive clinical team to deliver exceptional care
- Utilize dictation services to streamline documentation and maximize time for patient care
- Deliver hands-on nursing care, including acute & chronic disease management
- Manage patient medication
- Provide post-surgical rehabilitation support
- Administer pain management
- Perform wound care
Requirements
- Unencumbered Florida Registered Nurse license
- Valid driver’s license
- Auto insurance
- Reliable transportation
