This Registered Nurse position at a Community Living Center is responsible for delivering and supervising direct patient care, with an emphasis on patient education and self-management. The role involves coordinating care, administering medications and procedures, and collaborating with interdisciplinary teams to improve holistic care and patient outcomes.
Responsibilities
- Deliver and supervise direct patient care, fulfilling all aspects of the nursing process
- Oversee care coordination, focusing on patient education, self-management, and customer satisfaction
- Administer medications and procedures in accordance with established policies and guidelines
- Collaborate with interdisciplinary team members to influence and improve care outcomes
- Maintain knowledge of and actively participate in unit and program quality improvement initiatives
- Support customer service programs
- Provide competent, evidence-based care to assigned residents
- Supervise licensed vocational nurses and nursing assistants as appropriate
- Assist in directing nursing education, orientation, and competency development
- Participate in activities and initiatives aimed at fostering an evidence-based practice environment
- Collaborate with the Minimum Data Set (MDS) coordinator for accurate documentation and RUG scoring
- Act as a professional advocate for patients, families, and caregivers within the practice setting
Requirements
- English Language Proficiency
- Graduate of an ACEN or CCNE approved school of professional nursing
- Current, full, active, and unrestricted registration as a graduate professional nurse in a US State, Territory, Commonwealth, or DC
Qualifications
- Graduate of a school of professional nursing approved by ACEN or CCNE, or a Master's level bridge program with equivalent coursework to a Bachelor's degree in Nursing
- Entry level. Preferred two years of registered nursing experience in long-term care, hospice, and medical-surgical settings.
