About This Role

This Registered Nurse position at a Community Living Center is responsible for delivering and supervising direct patient care, with an emphasis on patient education and self-management. The role involves coordinating care, administering medications and procedures, and collaborating with interdisciplinary teams to improve holistic care and patient outcomes.

Responsibilities

  • Deliver and supervise direct patient care, fulfilling all aspects of the nursing process
  • Oversee care coordination, focusing on patient education, self-management, and customer satisfaction
  • Administer medications and procedures in accordance with established policies and guidelines
  • Collaborate with interdisciplinary team members to influence and improve care outcomes
  • Maintain knowledge of and actively participate in unit and program quality improvement initiatives
  • Support customer service programs
  • Provide competent, evidence-based care to assigned residents
  • Supervise licensed vocational nurses and nursing assistants as appropriate
  • Assist in directing nursing education, orientation, and competency development
  • Participate in activities and initiatives aimed at fostering an evidence-based practice environment
  • Collaborate with the Minimum Data Set (MDS) coordinator for accurate documentation and RUG scoring
  • Act as a professional advocate for patients, families, and caregivers within the practice setting

Requirements

  • English Language Proficiency
  • Graduate of an ACEN or CCNE approved school of professional nursing
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a US State, Territory, Commonwealth, or DC

Qualifications

  • Graduate of a school of professional nursing approved by ACEN or CCNE, or a Master's level bridge program with equivalent coursework to a Bachelor's degree in Nursing
  • Entry level. Preferred two years of registered nursing experience in long-term care, hospice, and medical-surgical settings.
