About This Role

Join a dedicated primary care team in Cheshire, CT, as a Nurse Practitioner or Physician Assistant to provide comprehensive care. You will manage your own patient panel and collaborate with experienced providers in a supportive outpatient setting.

Responsibilities

• Perform routine physical examinations
• Formulate medical, educational, and psychosocial plans to treat patients
• Develop plans for further evaluation of health needs in consultation with physician(s) and care team members
• Provide appropriate treatment for problems within the scope of practice
• Identify problems outside of scope of practice and refer appropriately to physician

Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family, Adult/Geri Nurse Practitioner) or PA training
• 1+ years of experience in a primary care or related outpatient setting
