About This Role

This role involves providing direct, high-quality, patient-centered cardiac care in an outpatient setting, collaborating with physicians, and educating patients and their families on treatment plans and outcomes for complex cardiovascular conditions.

Responsibilities

  • Provide direct care to patients in the outpatient setting in collaboration with physicians
  • Provide high quality patient-centered care, advocacy and education for patients and their families
  • Communicate effectively with all members of the healthcare team, patients and family
  • Diagnose, treat and manage complex cardiovascular care including acute and chronic disease, medication management and other therapies
  • Identify diagnostic information that should be considered emergent and communicate timely information to physician colleagues

Requirements

  • Graduate of an accredited Physician Assistant or Nurse Practitioner Master’s Program
  • Licensed, or eligible to be licensed, as a Physician Assistant (PA-C), Family Nurse Practitioner (FNP), or Adult Geriatric Primary Care (AGPCNP) in the state of Connecticut
  • Strong clinical background and the ability to diagnose, treat and manage complex cardiovascular care
  • Ability to identify diagnostic information that should be considered emergent and communicate timely information to physician colleague
  • Self-motivated and flexible to meet the demands of the specialty
  • Exhibit a passion for cardiology

Qualifications

  • Master’s Program in Physician Assistant or Nurse Practitioner
  • Experienced Physician Assistant or Nurse Practitioner (APRN)
