Responsibilities

• Provide direct care to patients in the outpatient setting in collaboration with physicians
• Provide high quality patient-centered care, advocacy and education for patients and their families
• Communicate effectively with all members of the healthcare team, patients and family
• Educate patient and family members ensuring they understand care, testing, medical conditions, treatment regimens, and expected outcomes
• Perform clinical management in collaboration with physicians and hospital-based APPs
• Diagnose, treat and manage complex cardiovascular care including acute and chronic disease, medication management and other therapies
• Identify diagnostic information that should be considered emergent and communicate timely information to physician colleagues
• Cover other offices in the central region when needed

Requirements

• Graduate of an accredited Physician Assistant or Nurse Practitioner Master’s Program
• Licensed, or eligible to be licensed, as a Physician Assistant (PA-C), Family Nurse Practitioner (FNP), or Adult Geriatric Primary Care (AGPCNP) in the state of Connecticut
• Strong clinical background and the ability to diagnose, treat and manage complex cardiovascular care

Qualifications

• Master’s Program Graduate of an accredited Physician Assistant or Nurse Practitioner
• Experienced Physician Assistant or Nurse Practitioner (APRN)
