About This Role

Provide independent, age-appropriate physical therapy to home health and hospice patients in their place of residence. This role involves evaluating patients, establishing treatment plans, and coordinating care with other healthcare personnel.

Responsibilities

  • Evaluate patients and interpret evaluation findings to determine the nature and extent of dysfunction, including adaptive equipment needs and Activities of Daily Living (ADLs)
  • Establish treatment goals and plans to achieve them, including for adaptive equipment needs
  • Initiate discharge planning for physical therapy
  • Implement the plan of care through direct treatment and delegate portions to appropriate supportive personnel
  • Coordinate the treatment program with other health care personnel
  • Provide advice and education to patients and their families/caregivers
  • Regularly reassess clinical signs and symptoms to determine treatment effectiveness and progress towards goals
  • Assess for adaptive equipment for ADLs based on clinical guidelines and coordinate with certified equipment vendors
  • Supervise the work of Home Health Aides according to regulation and agency policy
  • Provide accurate, timely documentation of physical therapy patient management, including adaptive equipment assessment

Requirements

  • Physical Therapist license
  • Ability to make home visits/work in patients' residences
