About This Role

The Licensed Practical Nurse (LPN I) practices nursing under the supervision of a Registered Nurse or Physician, providing direct and indirect care to patients and families. This role involves educating patients, coordinating care, and collaborating with the healthcare team to ensure high-quality care.

Responsibilities

• Prepare patient for office visit by reviewing history, collecting relevant clinical information, and measuring vital signs, documenting in the medical record
• Complete POE and address care gaps during the patient encounter
• Prepare room and equipment for procedures and assist the practitioner as needed
• Initiate interventions and document response to treatment according to established workflows and guidelines
• Participate in emergency care as directed by practitioner or registered nurse
• Educate patients and provide preparation materials for diagnostic studies according to guidelines
• Schedule appointments for patients with lab, radiology, or specialty consultations as ordered by practitioner
• Monitor electronic messaging and emails (department InBasket) related to patient or provider requests and manage messages within scope of practice
• Participate in departmental or regional quality improvement activities and initiatives
• Engage patients, families, and caregivers in the coordination of care
• Resolve patient concerns promptly
• Provide departmental orientation and training of new team members on standard work, policies, and procedures
• Participate in the supervision of unlicensed personnel in the delivery of patient care
