About This Role

The Registered Nurse (RN) Navigator provides professional nursing care focusing on patient and family-centered care. This role educates and provides resources to facilitate informed decision-making and access to quality health and psychosocial care throughout the care continuum.

Responsibilities

  • Assist patients and families in understanding their diagnosis, treatment options, and available resources within Kaiser Permanente.
  • Perform assessment, symptom management, data analysis, planning, case management, and implementation of interventions for patients with chronic diagnoses.
  • Develop and facilitate a patient-centered plan of care in consultation with treating physicians, patients, and their support network.
  • Serve as a link between patients, healthcare providers, family, and support team, coordinating care across various settings.
  • Conduct care assessments and triage for new and ongoing patients via phone, clinic, or other communication methods.
  • Facilitate appointments for consults, procedures, and support services within established service lines.
  • Standardize patient education materials by developing and maintaining a resource library specific to the specialty patient population.
  • Deliver patient and family education prior to treatment and remain available for questions and concerns.
  • Monitor patient compliance with treatment plans and assist with referrals to outside resources and care coordination.
  • Collaborate with external healthcare members regarding inpatient treatment and discharge planning.
  • Develop a database of community resources for patients and act as a central point of contact for navigated patients involved in data collection or research projects.
  • Carry out direct patient care activities within scope of practice and accurately document care in the patient medical record.
  • Serve as a preceptor, resource, and mentor for new healthcare team members and nursing students.
  • Identify clinical quality indicators for relevant patient populations and conduct systematic reviews to implement and sustain changes in nursing practice.

Requirements

  • Registered Nurse (RN) license
  • Knowledge of Georgia Registered Professional Nurse Practice Act
  • Ability to develop and maintain high quality evidence-based care processes
