This role is responsible for coordinating patient care, managing the referral process, and ensuring seamless communication between staff, families, and external organizations for an early intervention day treatment clinic for children with special needs.
Responsibilities
- Initiate and maintain direct communications with various organizations (businesses, professional, social, charitable, public, religious, and private) to market clinic services to the community.
- Serve as liaison between staff, families, and outside sources for patient enrollment.
- Work closely with professional staff for patient monitoring and coordination of services.
- Assist in investigating patient absences and remedying problems.
- Facilitate patient access to community resources, including food, clothing, and school programs.
- Assist families with social service forms and paperwork (e.g., SSI applications, Medicaid, TEFRA, AR Kids First).
- Manage the referral process for potential and existing patients by coordinating with local physicians.
- Schedule evaluations for potential patients and complete the intake process.
- Maintain current EIDT Prior Authorizations (PAs) and request extensions/modifications when necessary.
- Perform chart reviews for quality assurance, requesting missing documentation and error corrections.
- Develop and maintain Multidisciplinary Treatment Plans (MTPs) for CHMS patients.
- Organize and coordinate audiology services, following up with PCP and parent/guardian as indicated.
- Monitor the daily well-being of patients and identify needs for social intervention.
- Lead Multidisciplinary Patient Reviews to update on therapy goals, progress, and family dynamics.
- Provide progress notes for each service billed, reflecting treatment plan goals, objectives, and current diagnoses.
- Respond to staff when a patient is identified at-risk and make efforts to correct or manage the identified problem.
Requirements
- Bachelor's degree in business, education, health care, or related field OR Associate's degree with a license to practice as a physical therapy assistant, occupational therapy assistant, speech-language pathologist assistant, RN or LPN
- One year of successful experience in a similar position
Qualifications
- Bachelor's degree in business, education, health care, or related field. Master's degree preferred. Associates degree with a license to practice as a physical therapy assistant, occupational therapy assistant, speech-language pathologist assistant, RN or LPN is permitted.
- One year of successful experience in a similar position or equivalent combination of education and experience.
