About This Role

This role involves coordinating patient care within a home-based team, managing communications, scheduling appointments, and ensuring accurate patient records to support comprehensive geriatric medical care.

Responsibilities

  • Answer incoming and outgoing calls, prioritizing patient concerns
  • Coordinate care with the medical team, including scheduling patient and specialist appointments
  • Coordinate diagnostic tests and act as a community resource for patients
  • Engage unengaged patients and contribute accurate data to patient records
  • Update and maintain digital client records, assist in transitioning hard copy records to digital format
  • Conduct patient scheduling, registration, and eligibility verification
  • Manage patient medical records as appropriate
  • Schedule and coordinate WellBe provider visits with patients, tests, specialist appointments, and social resources
  • Ensure all appropriate patient information is in the EMR
  • Prioritize urgent patient needs and scheduling for Complete Care Assessments
  • Collaborate with the interdisciplinary team and participate in team meetings
  • Support APC in ongoing panel management and quality of care efforts (e.g., HEDIS)
  • Complete next day visit chart prep
  • Utilize reporting to help providers track, meet, and exceed team goals

Requirements

  • High school graduate or GED
  • 2+ years of experience working for a healthcare practice or hospital
  • High level of professionalism
  • Strong critical thinking skills
  • Strong customer centric focus/service skills
  • Strong computer skills, including Word, Excel, and PowerPoint
  • Strong verbal, written, presentation, and interpersonal communication skills

Qualifications

  • High school graduate or GED
  • 2+ years of experience working for a healthcare practice or hospital

Nice to Have

  • MA Certification
  • EMR documentation experience
