The Patient Care Coordinator is a vital member of a home-based care team, providing compassionate support to frail, elderly patients with chronic and acute illnesses. This role involves coordinating care, managing appointments, and ensuring accurate patient records to facilitate complete care delivery.
Responsibilities
- Answer incoming and outgoing calls, prioritizing patient concerns appropriately
- Coordinate care with the medical team, including scheduling patient and specialist appointments
- Schedule diagnostic tests and ensure all appropriate patient information is captured in the EMR
- Act as a community resource and engage unengaged patients
- Manage patient medical records and assist in transitioning to digital format
- Provide positive customer service and treat all patients and staff with respect
- Collaborate with the interdisciplinary team and participate in regular meetings
- Support APCs in panel management and quality of care efforts (e.g., HEDIS)
- Complete next day visit chart preparation and utilize reporting to track team goals
Requirements
- High school graduate or GED
- 2+ years of experience working for a healthcare practice or hospital
- High level of professionalism
- Strong critical thinking skills
- Strong customer centric focus/service skills
- Strong computer skills (Word, Excel, PowerPoint)
- Strong verbal, written, presentation, and interpersonal communication skills
Qualifications
- High school graduate or GED
- 2+ years of experience working for a healthcare practice or hospital
Nice to Have
- MA Certification
- EMR documentation experience
