Provide direct, one-on-one medical care to medically fragile infants, children, and adolescents primarily in their homes, focusing on medical needs and treatment according to the active plan of care.
Responsibilities
- Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care
- Conduct on-going patient care and assessments
- Administer prescribed medication, treatments, and therapies
- Coordinate care
- Educate family members on patient clinical care to enhance positive outcomes
- Implement preventative initiatives to protect quality of care for patient
Requirements
- Active RN or LPN/LVN license
- Ability to provide care in a client home setting
