About This Role

This role is responsible for managing a caseload of clients experiencing homelessness, ensuring their clinical files and reports are current, and providing client-centered services to help them secure entitlements, employment, and housing.

Responsibilities

  • Manage all aspects of an assigned client caseload
  • Meet with clients on a regular basis
  • Ensure all clinical files and reports are complete and up to date
  • Engage individuals in a client-centered approach
  • Provide services and referrals for individuals experiencing homelessness
  • Assist clients in securing entitlements, gainful employment, and housing
  • Manage conflict or challenging behaviors with self-awareness, emphasizing client autonomy and safety
  • Build clients' daily living skills for successful independent living
  • Complete intakes and assessments and all required documentation (psychosocials, progress notes, treatment plans)
  • Escort clients to various appointments, as needed
  • Participate in interdisciplinary team meetings and case conferences
  • Provide emergency first aid/CPR assistance when needed

Requirements

  • High School Diploma/GED
  • Proficiency in MS Office (Word, Excel)
  • CPR training certification or willingness to complete training

Qualifications

  • High School Diploma/GED required, Bachelor's Degree preferred

  • Experience working with individuals experiencing homelessness
  • Experience with conducting outreach, securing entitlements and housing
  • Knowledge of mental illness and substance abuse
