About This Role

This role involves providing intake, assessment, and counseling for homeless mentally ill clients. The Independent Living Specialist will offer concrete services, advocacy, and support housing placement, contributing to clients' health and self-sufficiency.

Responsibilities

  • Conduct intake and assessment of clients
  • Provide individual and group counseling
  • Offer concrete services, entitlements, and advocacy
  • Escort clients to various appointments
  • Perform documentation, record keeping, and progress notes
  • Engage in psychosocial support, treatment planning, and aftercare follow-up
  • Attend staff meetings and trainings
  • Facilitate housing placement
  • Provide emergency first aid/CPR assistance when needed

Requirements

  • High School Diploma
  • Proficiency in MS Office, including Word, Excel

Qualifications

  • High School Diploma required. BA preferred.
  • Experience working with homeless mentally ill. Outreach, entitlement, substance abuse experience helpful.
