Hybrid Primary Care - Family Nurse Practitioner or Physician Assistant

About This Role

This role is for a Family Nurse Practitioner or Physician Assistant to provide primary care services in a hybrid-remote setting, leveraging state-of-the-art technology to mostly conduct remote same-day visits while also handling in-person duties at two Campus Care locations.

Responsibilities

• Provide primary care services for same day visits remotely
• Provide primary care services at two Campus Care locations in-person
• Perform routine physical examinations
• Formulate medical, educational, and psychosocial plans to treat patients
• Develop plans for further evaluation of health needs in consultation with physicians and care team members
• Provide appropriate treatment for problems within the scope of practice
• Identify problems outside of scope of practice and refer appropriately to physician

Requirements

• Unrestricted State of Connecticut Medical License (or eligible)
• APRN (Family Nurse Practitioner) or PA training
• 5+ years of post-graduate experience as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) in a primary care setting

Qualifications

• 5+ years of post-graduate experience as a Family Nurse Practitioner (FNP) or Physician Assistant (PA) in a primary care setting.
