Full Time Hospice RN Case Manager

About This Role

This role is for a compassionate and skilled Registered Nurse Case Manager who will coordinate care, develop individualized plans, and provide hands-on hospice and palliative care for patients and families facing life-limiting illnesses. The position covers San Marcos, Vista, Oceanside, Encinitas, and Solana Beach areas.

Responsibilities

• Develop detailed Plans of Care for assigned patients, ensuring all services are identified and met
• Collaborate with the Interdisciplinary Group, patient, family, and attending physician to coordinate care
• Provide skilled hospice and palliative care for the remainder of the patient’s life
• Conduct comprehensive nursing evaluations and assessments
• Provide field supervision to LVNs and Home Health Aides assigned to your patients
• Perform field case management

Requirements

• Registered Nurse with a valid state license
• Minimum of 1 year experience as an RN
• Ability to lift up to 50 pounds
• Valid driver’s license, good driving record, and reliable transportation

Qualifications

• Minimum of 1 year experience as an RN required

Nice to Have

• Hospice experience preferred
• Bilingual English/Spanish
