This role involves planning, organizing, and directing hospice care for patients and families, leveraging community resources to provide comprehensive services in their homes. The Hospice RN Case Manager will assess patient needs, develop individualized care plans, and coordinate with interdisciplinary teams to ensure continuity of care.
Responsibilities
- Complete initial, comprehensive, and ongoing assessments of patient and family hospice needs, including physical assessment and history of illnesses
- Provide professional nursing care utilizing all elements of the nursing process as defined in the state Nurse Practice Act
- Initiate and revise the plan of care based on health assessment data and patient status changes, incorporating palliative nursing actions
- Administer medications and treatments as prescribed by the physician in the physician’s plan of care
- Counsel patients and families in meeting nursing and related needs and provide healthcare instructions
- Act as Case Manager, coordinating patient care for an assigned caseload
- Complete, maintain, and submit accurate clinical notes regarding patient’s condition and care given
- Communicate with physicians regarding patient needs and changes in condition, obtaining/receiving orders as required
- Teach patients and families/caregivers self-care techniques, medication, and diet instructions
- Ensure arrangements for equipment and other necessary items and services are available
- Supervise ancillary personnel and delegate responsibilities when required
- Participate in quality assessment performance improvement teams and activities
Requirements
- 2 years' RN experience in healthcare (preferably home care or hospice)
- Graduate of an accredited school of nursing
- Current Minnesota Nursing License
- Pass a MVR check based on Lifespark company policy

Nice to Have
- Experience in customer service/direct member contact field
- Experience supervising caregivers
- Knowledge of corporate business management, governmental regulations, ACHC standards, and private payer practice
