Patient Care Manager (RN)

About This Role

Lead clinical operations, support interdisciplinary teams, and ensure high standards of patient care for hospice patients. Maintain compliance, foster collaboration, and provide hands-on support in a management role.

Responsibilities

• Manage clinical and operational responsibility for your team
• Serve as liaison between the Director of Patient Care Services, clinical staff, patients, families, and the community
• Oversee scheduling, workflow, and compliance for clinical associates
• Provide routine patient care in the absence of other clinical staff members
• Ensure adherence to hospice policies, state and federal regulations, and Silverado standards
• Support quality improvement initiatives and interdisciplinary team collaboration

Requirements

• Registered Nurse with a valid state license
• Minimum of 1 year experience in healthcare management, ideally in hospice
• Valid driver’s license and auto insurance
• Reliable transportation and willingness to drive to patient locations
• Ability to lift up to 50 pounds
