Community Health Nurse

About This Role

This per-diem Community Health Nurse role involves providing, teaching, supervising, and documenting nursing care for the sick and injured in their homes. The nurse will collaborate with other healthcare providers to coordinate care and evaluate patient responses.

Responsibilities

• Provide nursing care to the sick and injured at home
• Teach and supervise nursing care
• Document nursing care and patient responses
• Promote, maintain, or restore health and prevent illness
• Effect rehabilitation for patients
• Collaborate with other healthcare providers and community representatives to coordinate care
• Evaluate individual and family responses to interventions

Requirements

• Current RN license in Massachusetts
• Active and valid driver's license
• CPR certification
• Demonstrated knowledge of principles and practices of nursing
• Knowledge of and ability to apply community health principles and practice
• Ability to perform independently as a clinician with minimal supervision or assistance
• Ability to exercise independent judgement
• Demonstrated interpersonal skills
• Ability to supervise others
• Ability to maintain confidential information
• Ability to develop and maintain records and communicate clinical data in verbal and written form
• Willing to be flexible regarding work hours
• Ability to drive an automobile in inclement weather
• Willing to accept “on call” responsibilities
• History of dependability in meeting demands of committed work schedule

Qualifications

• Bachelor of Science in Nursing recommended; Graduate of a N.L.N. accredited nursing program
• One-year acute nursing care preferred, hospital based preferred; Experience in home care setting strongly preferred
