The Clinical Pharmacist for Pediatrics at Helen DeVos Children's Hospital is responsible for ensuring the safety and efficacy of medication use, providing education to patients and healthcare teams, and participating in process improvement activities. This role involves interpreting medication orders, evaluating patients, and providing dosage regimen recommendations.
Responsibilities
- Maintain accountability for ensuring the safety and efficacy of medication use.
- Educate and act as a consultant to patients/family and members of the health care team related to optimal drug therapy.
- Participate in process improvement activities and drug use evaluations.
- Interpret and screen medication orders for appropriate agent, dose, route, allergies, and potential drug interactions.
- Conf safely and dispense medications for administration to patients.
- Evaluate patients, order appropriate laboratory tests, and interpret serum drug levels utilizing pharmacokinetic principles.
- Provide prescribers with dosage regimen recommendations to optimize therapeutic outcomes, reduce patient risk, and monitor the costs associated with drug therapy.
- Provide appropriate supervision and direction to technicians, residents, interns, and other support personnel.
- Engage in continuous study of the entire professional field to maintain professional competence, knowledge, and skills.
- Provide education and counseling to patients/family/significant others/members of the health care team.
- Ensure that services are provided in accordance with State and Federal regulations, organizational policy, and accreditation/compliance requirements.
Requirements
- Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
- Clinical Pharmacist - State of Michigan license (within 90 days)
- Pharmacist Controlled Substance - State of Michigan license (within 90 days)
- ACLS, PALS, or BLS certification (within 180 days)
Qualifications
- Bachelor's Degree Pharmacy or Doctor of Pharmacy degree from an accredited College of Pharmacy
Nice to Have
- 2 years of relevant experience in hospital pharmacy
- American Society of Health-System Pharmacists (ASHP) accredited residency in Pharmacy Practice
- PharmD and either PGY1 or PGY2 Residency in Pediatrics
- Inpatient Pharmacy experience
- Pediatric Pharmacy experience
