The Clinical Pharmacist provides safe and appropriate pharmacy services for home care patients, assessing their suitability for home care and designing and monitoring their pharmacotherapy plans. This role involves educating patients, caregivers, and staff on medication use and ensuring continuity of pharmaceutical care.
Responsibilities
- Determine the suitability of individual patients for home care by collecting and organizing patient-specific information.
- Design, recommend, monitor, and evaluate patient-specific pharmacy care plans for home care patients.
- Formulate and make appropriate recommendations for modifications in prescribed medication therapy or monitoring parameters.
- Provide medication-use education to Option Care staff, other professionals, home care patients, and caregivers.
- Ensure continuity of pharmaceutical care to and from the home and other patient-care settings.
- Prepare and dispense medications using appropriate techniques and adhering to safety and quality assurance practices.
- Manage the use, maintenance, and troubleshooting of medication administration and related equipment.
- Provide concise, applicable, and timely responses to requests for drug information from healthcare providers and home care patients.
- Manage the use of investigational drug products according to regulations and policies.
- Utilize pharmacy support personnel effectively, prioritize workload, and ensure accuracy of their work.
Requirements
- Graduate of an accredited School of Pharmacy
- Current, active Pharmacy license as granted by the State Board of Pharmacy
- At least 1 year of experience in sterile IV admixture procedures OR knowledge of non-sterile compounding, immunizations and medication therapy management (MTM) OR completion of an ASHP accredited Pharmacy Residency Program OR completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy OR equivalent experience
- Experience providing customer service to internal and external customers
- Basic Microsoft Word skills
- Basic Microsoft Excel skills
- Basic Microsoft PowerPoint skills
Qualifications
- Graduate of an accredited School of Pharmacy
- Entry level position, with at least 1 year of experience in sterile IV admixture procedures OR knowledge of non-sterile compounding, immunizations and MTM OR completion of an ASHP accredited Pharmacy Residency Program OR completion of Advanced Pharmacy Practice Experience
Nice to Have
- Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy
- Experience in home care, hospital or other clean room setting
