About This Role

The Clinical Pharmacy Specialist contributes to cancer treatment by providing safe, evidence-based, and patient-centered medication management. This role supports the Malignant Hematology interdisciplinary team by offering clinical pharmacy services and optimizing patient outcomes through pharmaceutical care plans.

Responsibilities

  • Provide oversight for safe and appropriate medication prescribing, dispensing, administering, and monitoring
  • Deliver clinical pharmaceutical care to optimize drug therapy for cancer patients
  • Perform medication order review, clarification, and verification
  • Supervise accurate hazardous and non-hazardous drug preparation and distribution
  • Process and double-check chemotherapy orders as necessary
  • Develop, interpret, and provide medication-related information and education to healthcare personnel
  • Coordinate and provide medication therapy education and counseling to patients and caregivers
  • Precept pharmacy students, residents, and others in training
  • Participate in clinical research and projects
  • Provide leadership for developing, implementing, and maintaining medication-related guidelines, policies, and standards

Requirements

  • Doctor of Pharmacy Degree
  • Board of Pharmacy Specialties (BPS) certification
  • Experience in medication management
  • Experience in oncology
  • Experience in malignant hematology
  • Ability to develop and implement pharmaceutical care plans
  • Knowledge of medication safety
  • Knowledge of formulary management
  • Knowledge of policy development

Qualifications

  • Doctor of Pharmacy Degree

Nice to Have

  • PGY-2 Residency in Hematology/Oncology
  • Experience in chemotherapy order processing
  • Experience in clinical research
  • Skill in education of patients, family members, healthcare providers, and pharmacy students and residents
