The Clinical Pharmacy Specialist contributes to cancer treatment by providing safe, evidence-based, and patient-centered medication management. This role supports the Malignant Hematology interdisciplinary team by offering clinical pharmacy services and optimizing patient outcomes through pharmaceutical care plans.
Responsibilities
- Provide oversight for safe and appropriate medication prescribing, dispensing, administering, and monitoring
- Deliver clinical pharmaceutical care to optimize drug therapy for cancer patients
- Perform medication order review, clarification, and verification
- Supervise accurate hazardous and non-hazardous drug preparation and distribution
- Process and double-check chemotherapy orders as necessary
- Develop, interpret, and provide medication-related information and education to healthcare personnel
- Coordinate and provide medication therapy education and counseling to patients and caregivers
- Precept pharmacy students, residents, and others in training
- Participate in clinical research and projects
- Provide leadership for developing, implementing, and maintaining medication-related guidelines, policies, and standards
Requirements
- Doctor of Pharmacy Degree
- Board of Pharmacy Specialties (BPS) certification
- Experience in medication management
- Experience in oncology
- Experience in malignant hematology
- Ability to develop and implement pharmaceutical care plans
- Knowledge of medication safety
- Knowledge of formulary management
- Knowledge of policy development
Qualifications
- Doctor of Pharmacy Degree
Nice to Have
- PGY-2 Residency in Hematology/Oncology
- Experience in chemotherapy order processing
- Experience in clinical research
- Skill in education of patients, family members, healthcare providers, and pharmacy students and residents
