The Case Manager delivers comprehensive case management services to individuals and families in Los Angeles and Inland Empire, focusing on supporting clients through critical transitions such as incarceration or homelessness. This role addresses mental, emotional, and social well-being through individualized assessments, counseling, and linkage to community resources. As part of a mobile and community-based health program, the Case Manager brings high-quality health support, preventive care, and education directly into the community.
Responsibilities
- Lead and supervise Community Health Workers (CHWs) and Certified Peer Support Workers (CPSW) by providing guidance, mentorship, and performance support.
- Oversee daily field operations of the Mobile Health Clinic, including logistics, deployment scheduling, and supply management.
- Conduct case management, assessments, and follow-ups for individuals with health and social service needs.
- Coordinate care across systems—linking patients to medical, health, and social support resources within their communities.
- Provide peer support services, drawing on personal experiences to build rapport and trust.
- Advocate for health needs by assisting patients in effectively communicating with healthcare providers or social service agencies.
- Implement programs that promote, maintain, and improve individual and overall community health.
- Conduct outreach activities within specific health programs and engage with local businesses to promote community health services.
- Provide health coaching, patient navigation, health education, and health promotion for a diverse panel, utilizing motivatonal interviewing and trauma-informed care.
- Assist patients in accessing health-related services, overcoming barriers to medical care, and utilizing community services.
- Establish positive, supportive relationships with patients and provide feedback to other team members.
- Manage monthly and quarterly report requests from local, state, and federal entities.
Requirements
- Violence Prevention Professional Certificate OR Work Experience Pathway
- 5 years of relevant professional experience or lived experience
- Ability to work independently and collaborate with teams
- Demonstrated knowledge of local and regional community resources
- Demonstrated knowledge of public health programs
- Skilled in utilizing appropriate industry standard assessment techniques
- Comprehensive knowledge of the local community based on personal lived experience or ability to articulate the lived experience
Qualifications
- Degree in Social Work, Counseling, Psychology, or a related behavioral health field
- Minimum 3-5 years of experience providing case management or health services, preferably within community-based, mobile, or integrated healthcare settings.
Nice to Have
- Certification as a Community Health Worker (CHW) and/or Certified Peer Support Worker (CPSW)
- Lived or professional experience connecting to the population served
- Familiarity with the geographic communities served by the mobile school health clinic
