Case Manager

About This Role

This hybrid Case Manager position involves providing information and education on the Medicaid waiver process, developing individual support plans, and facilitating community-based team meetings for individuals with intellectual and developmental disabilities in Floyd County, Indiana. It requires travel to community meetings and remote work from a home office.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Requirements

• Valid driver's license, car insurance, and reliable transportation
• Designated home office setting with computer, internet service, and smart phone
• Bachelor’s Degree in Human Services
• Minimum one year of experience working with Individuals with intellectual and developmental disabilities
• Reside within the State of Indiana or within 10 miles of the state line
