Case Manager

About This Role

This hybrid Case Manager position involves working with individuals with intellectual and developmental disabilities in DeKalb County, Indiana, focusing on Medicaid waiver processes and support plan creation. The role combines remote work with community-based meetings, requiring travel within a one-hour radius.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals served
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Requirements

• A valid driver's license
• Car insurance
• Reliable transportation
• Bachelor’s Degree in Human Services
• Minimum of one year of experience working with Individuals with intellectual and developmental disabilities
• Designated home office setting with computer, internet service, and smart phone
• Reside within Indiana or within 10 miles of the state line
