Case Manager

About This Role

This Case Manager position coordinates interdisciplinary care for designated patient populations, ensuring proper utilization of hospital resources and appropriate discharge planning.

Responsibilities

• Coordinate interdisciplinary care for a designated patient population
• Perform reviews of all inpatient admission records to ensure proper utilization of hospital resources
• Determine admission for appropriate level of care
• Assess and identify discharge needs
• Coordinate appropriate discharge plan
• Work collaboratively with the interdisciplinary team to facilitate achievement of desired financial and quality outcomes
• Maintain and enhance payor relationships

Requirements

• Licensure in the state where the hospital resides
• Current BLS certification
• 3+ years of inpatient experience

Qualifications

• Licensure in the state where the hospital resides is preferred. Professional disciplines of the designated case manager may include occupational therapist, physical therapist, registered nurse, respiratory therapist, speech therapist, or (licensed) social worker.
• Three (3) to five (5) years of inpatient experience

Nice to Have

• Experience in an acute, IRF, or LTACH setting
