This Case Manager role coordinates health care decisions to improve patient outcomes, manages care across the continuum, and ensures cost-effective care within the allotted time frame. The position also focuses on assuring payments for hospital services and implementing safe discharge plans, while assessing psychosocial needs as part of the discharge process.
Responsibilities
- Coordinate health care decisions using a systematic approach to assure treatment plans improve quality and outcomes
- Coordinate care across the continuum
- Promote cost-effective care within the allotted time frame
- Assure payments of hospital-based services meeting patient-related utilization management criteria
- Implement safe and appropriate discharge plans
- Assess the psychosocial needs of the patient and provide intervention as part of the discharge planning process
- Perform clinical interventions and discharge planning
- Manage care plans
- Handle fiscal management
- Manage payer and referral processes
Requirements
- Current Licensure per state guidelines in clinical or related discipline OR a Bachelor's or Master's in a health or human services discipline
Qualifications
- Current Licensure per state guidelines in clinical or related discipline (RN Preferred) OR a Bachelor's or Master's in a health or human services discipline
Nice to Have
- Previous experience in Case Management and Discharge Planning
- CCM Certification
