About This Role

The Case Manager will assist elderly clients in Brooklyn, NY with managing their healthcare and social service needs by coordinating services, advocating for them, and providing comprehensive case management.

Responsibilities

  • Coordinate day-to-day on-site services for clients on their caseload
  • Conduct in-home assessments and reassessments with elderly clients
  • Develop comprehensive care plans including assessment of informal and formal supports
  • Provide comprehensive ongoing case management services including information, case assistance, entitlement counseling, referral, supportive contact, advocacy, and crisis intervention
  • Prepare and maintain up-to-date records and client case files, unit entries, and outcomes
  • Accurately maintain client records and enter necessary service information in the computerized client database
  • Work collaboratively with internal team and community partners to coordinate homecare activities
  • Initiate referrals and coordinate deliveries with homebound meal agencies
  • Participate in all recommended and required training, such as mandatory DFTA training

Requirements

  • Bachelor’s Degree in social work, psychology, or a related human services field of study
  • Bilingual English/Mandarin speaker

Qualifications

  • Bachelor’s Degree in social work, psychology, or a related human services field of study required, Master in Social Work preferred

Nice to Have

  • Master in Social Work
  • Experience in providing EISEP case management or social services to elderly clients
  • Familiarity with entitlement and benefits and procedures for applications, especially in working with elderly clients
