About This Role

This role involves providing comprehensive case management services to formerly homeless Veterans and their families in Montgomery County, utilizing Harm-Reduction and Trauma-Informed Care principles. The Case Manager will assess client needs, implement service plans, and coordinate with various service providers to ensure clients maintain housing and achieve stability.

Responsibilities

  • Assess clients’ needs and appropriate services, implementing service plans and coordination
  • Maintain an active caseload of up to 16 Veterans or Veteran families in scattered site apartments
  • Provide a variety of client services including counseling, life skills training, medication monitoring assistance, and transportation
  • Participate in meetings with other service providers to resolve conflicts and monitor client progress
  • Maintain clients’ files and active resource files, completing all pertinent program paperwork
  • Complete client data for reporting purposes, including accurate entry into the Homeless Management Information System (HMIS) Service Point
  • Assess clients in crisis, resolve conflicts, and develop interventions for problematic behavior
  • Serve as ongoing liaison between property managers and clients, providing mediation and advocacy with landlords
  • Act as a representative of MCCH in collaboration with other service providers

Requirements

  • BA or BS in Social Work or Human Services field
  • Minimum of two years of experience in social and human services or related field
  • Hands-on experience in social and human services field dealing with diverse populations
  • Knowledge of principles, methods, and procedures of case management
  • Knowledge of Harm-Reduction and Trauma Informed Care
  • Knowledge of principles, methods, and procedures in handling addiction and dually diagnosis populations
  • Ability to negotiate and maintain positive relationships with co-workers and clients

Qualifications

  • BA or BS in Social Work or Human Services field
  • Minimum of two years of experience in social and human services or related field; hands-on experience dealing with diverse populations, with desired experience working with the homeless population.
