About This Role

This role involves evaluating and processing medication orders, preparing intravenous solutions and chemotherapy, and serving as a key resource for medication-related questions within a healthcare setting.

Responsibilities

  • Evaluate medication orders based on patient's weight, allergy history, and compatibilities, properly recording them in the patient profile
  • Complete medication orders without error and ensure they are ready for delivery within specified times
  • Clarify and resolve problem orders, contacting physicians or nurses as needed and relaying information to the next shift pharmacist
  • Detect all problem medication orders, generate appropriate alternatives, and effect change in patient care
  • Train other pharmacists and technicians in unit dose and Intravenous (IV) areas
  • Prepare all IV preparations (TPN, central lines, antibiotics, chemotherapy) and respiratory therapy syringes in accordance with departmental guidelines
  • Check for errors in IV preparations and ensure delivery according to policy
  • Process Chemotherapy orders and ensure dosages and calculations are checked by two Pharmacists
  • Use currently accepted precautions when preparing chemotherapy (gowns, gloves, chemotherapy mat) and prepare it in the appropriate hood using indicated calculations
  • Serve as an informational resource for the institution, using current references to answer medication-related questions (drug compatibilities, interactions, dosage, etc.)
  • Review patient data upon physician request to make appropriate recommendations on serum concentration timing for various medications
  • Handle questions and problems professionally, anticipating and analyzing conflict situations to take preventative or corrective action
  • Identify concerns to appropriate persons and assist in problem solving and decision making
  • Function in any area in the central pharmacy where a pharmacist is needed and take administrative initiative to handle problems when a supervisor is not available

Requirements

  • Bachelor of Science in Pharmacy or PharmD Degree
  • Pharmacist License in the state of Ohio or license within 3 months of hire
