Perform psychosocial assessments, provide resource support and psychosocial interventions, and collaborate with interdisciplinary teams to promote patient and family well-being within a hospital setting.
Responsibilities
- Perform psychosocial assessments, considering family, social, and medical history
- Maintain current knowledge of medical and mental health diagnoses
- Report suspicions of abuse and neglect to legally mandated authorities
- Collaborate with the healthcare team to develop treatment plans and identify appropriate resources
- Maintain fluent knowledge of internal and external resources
- Provide supportive counseling to improve emotional coping and adherence to treatment plans
- Educate patients, families, caregivers, and healthcare team members about treatment options
- Provide self-management support to high-risk/complex patients/families
- Coordinate and facilitate family-centered care team meetings
- Provide crisis assessment and interventions to patients, families, and caregivers
- Advocate for the needs of patients/families/caregivers within the healthcare system and community
- Facilitate communication and coordination between members of the health care team and patient/family/caregiver
- Educate healthcare professionals and students on psychosocial aspects of patient care and the role of social work
- Participate in professional growth opportunities and attend meetings/conferences
Requirements
- Master's degree in Social Work
- Licensed Social Worker (LSW) in the State of Ohio (licensure must be secured by 4/1/2026)
