Social Work Fellow

About This Role

Perform psychosocial assessments, provide resource support and psychosocial interventions, and collaborate with interdisciplinary teams to promote patient and family well-being within a hospital setting.

Responsibilities

• Perform psychosocial assessments, considering family, social, and medical history
• Maintain current knowledge of medical and mental health diagnoses
• Report suspicions of abuse and neglect to legally mandated authorities
• Collaborate with the healthcare team to develop treatment plans and identify appropriate resources
• Maintain fluent knowledge of internal and external resources
• Provide supportive counseling to improve emotional coping and adherence to treatment plans
• Educate patients, families, caregivers, and healthcare team members about treatment options
• Provide self-management support to high-risk/complex patients/families
• Coordinate and facilitate family-centered care team meetings
• Provide crisis assessment and interventions to patients, families, and caregivers
• Advocate for the needs of patients/families/caregivers within the healthcare system and community
• Facilitate communication and coordination between members of the health care team and patient/family/caregiver
• Educate healthcare professionals and students on psychosocial aspects of patient care and the role of social work
• Participate in professional growth opportunities and attend meetings/conferences

Requirements

• Master's degree in Social Work
• Licensed Social Worker (LSW) in the State of Ohio (licensure must be secured by 4/1/2026)
