About This Role

Join our dynamic team as a Licensed Social Worker at Vantage Care, LLC. You will be responsible for providing social services to residents in a nursing home or rehabilitation center, ensuring compliance with federal, state, and local guidelines.

Responsibilities

• Plan, organize, develop, and implement all required social services to meet the needs of residents in accordance with federal, state, and local guidelines that govern skilled nursing facilities
• Perform all documentation requirements for social service documentation in a timely fashion
• Complete all admission assessments and evaluations
• Work with the interdisciplinary team to create a plan of care for residents
• Work with and provide support to residents, families, and staff
• Attend weekly Care Plan and Medicare meetings
• Receive and attempt to resolve resident grievances
• Coordinate resident discharge planning with all relevant departments and assist with scheduling outside services
• Participate in monthly team meetings such as QA and Safety

Requirements

• Experience as a social worker within a nursing home or rehabilitation center strongly preferred
• Knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities
• MA Social Work License (Required)

Qualifications

• 1+ year experience as a social worker within a nursing home or rehabilitation center; 1+ year experience in admissions and long term care preferred

Nice to Have

• 1 year experience in Admission
• 1 year experience in long term care
