The Hospice Social Worker provides comprehensive psychosocial assessments and serves as the mental health professional for the hospice interdisciplinary team. This role offers appropriate interventions to meet the complex needs of customers and families in community settings, primarily focusing on assisting them in coping with and preparing for end-of-life.
Responsibilities
- Serve as an integral member of a self-directed hospice interdisciplinary team
- Provide comprehensive psychosocial assessments
- Serve as the mental health professional for the hospice team
- Provide appropriate interventions to meet the complex needs of customers and families in community settings
- Assist customers and families in coping with and preparing for end of life
Requirements
- Masters or Doctoral degree from an accredited school of social work
- Licensure at the LICSW level in the state of Minnesota
Qualifications
- Masters or Doctoral degree from an accredited school of social work
Nice to Have
- 4000 hours of post-graduate clinical social mental health professional experience (if required to psychiatrically diagnose or engage in psychotherapy)
- Hospital social work experience
- Palliative Care experience
- Experience with Serious Persistent Mental Illness (SPMI) population/mental health diagnoses (for psychiatric social work)
- Knowledge of current case management principles, length of stay management, and/or transition/discharge planning
