About This Role

This contract Medical Social Worker role supports terminally ill patients and their families in home, community, and inpatient hospice settings, focusing on psychosocial assessments, care planning, and coordinating resources. The position primarily covers a specific geographic area within Vermont.

Responsibilities

• Provide psychosocial assessments, counseling, and care planning for hospice patients and families
• Participate in goals-of-care discussions and interdisciplinary team meetings
• Support complex symptom management and end-of-life planning
• Coordinate community resources and services
• Provide caregiver education and emotional support
• Maintain timely and accurate clinical documentation
• Assist with coverage across home hospice, inpatient hospice, and community hospice as needed

Requirements

• Master’s Degree in Social Work (MSW)
• 2+ years of relevant experience
• Vermont LMSW licensure
• BLS certification
• Recent Epic experience (within the past 24 months)

Qualifications

• Minimum 2+ years of relevant experience (3+ years preferred)
