About This Role

Provide emotional support, crisis intervention, and counseling for pediatric, adolescent, adult, and older adult patients and their families. This role involves case management, psychosocial assessments, and coordinating discharge planning within a multidisciplinary team.

Responsibilities

  • Provide emotional support, crisis intervention, and counseling for patients across all age groups and their significant others
  • Offer information and referral services
  • Refer for financial assistance and supportive services for patients and families with problems related to hospitalization, illness, disability, or trauma
  • Provide case management and milieu management
  • Complete psychosocial assessments
  • Assist in the development and re-evaluation of Treatment Plans
  • Work as a member of a multidisciplinary team
  • Coordinate discharge planning
  • Complete necessary documentation and maintain clinical records for each client

Requirements

  • Master's Degree in Social Work or Marriage and Family Therapy or related field
  • Current registration with State Board required within 3 months of hire (if no licensure required)
  • License required at time of hire and must be maintained (if licensure required)
  • Behavioral Violence Prevention Training within 3 months of hire and maintain current
  • Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care
  • Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.)

Qualifications

  • Master's Degree in Social Work related field OR Master's Degree in Marriage and Family Therapy or related field
  • Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care. Experience in working with a diverse population with a wide range of problems.

Nice to Have

  • Current BCLS (AHA) certificate upon hire and maintain current
