This Social Worker I role involves coordinating patient-centered care and service delivery in collaboration with physicians and interdisciplinary teams. The position focuses on facilitating optimal transitions and progression in care within a healthcare setting, ensuring quality patient outcomes.
Responsibilities
- Demonstrate values and standards of the social work profession and participate as part of the Case Management team to support in a collaborative effort to facilitate quality patient care
- Integrate and coordinate various aspects of service delivery, care facilitation, service access, and discharge and post-discharge activities
- Complete an initial screen of all assigned patients to identify readmission risks, patient strengths and needs related to clinical resource utilization and transition planning
- Initiate, complete and document transition planning for patients in assigned caseload
- Communicate necessary information to stakeholders and educate patients and families
- Plan effectively to meet patient needs, manage length of stay, and promote efficient utilization of resources (fiscal, human, environmental, equipment and services)
- Advocate for the safe discharge of patient to home or community resource, and support preventive and follow-up care post-discharge
- Maintain reliable systems to document, track, and monitor assigned cases
Requirements
- Master's Degree Social Work
- 1-3 years of Social Work experience
- Licensed Master Social Worker Tennessee
- Excellent interpersonal skills
- Comprehensive knowledge of community resources
- Knowledge of and ability to gather relevant data to synthesize and summarize information in making judgments regarding patient care
- Excellent oral and written communication skills
- Ability to establish constructive relationships with patient and families and manage difficult social situations
- Ability to organize multiple tasks and projects and maintain control of one’s own work flow
- Ability to use initiative in decision-making; independent judgment and critical thinking skills
- Demonstrated ability to develop and maintain working relationships with physicians and work collaboratively with health professionals at all levels to achieve established goals
- Demonstrated excellent facilitation skills
- Proficient in basic word processing skills (Word), internet navigation, and electronic medical record
